Không có tĩnh mạch cửa bẩm sinh, còn được gọi là dị tật Abernethy, là một dị tật hiếm gặp do loạn sản của hệ thống tĩnh mạch cửa. Có một số báo cáo về ghép gan như là một phương pháp điều trị cho dị tật Abernethy và báo cáo của chúng tôi là trường hợp đầu tiên ở Việt Nam. Ca bệnh: Bệnh nhân nam 10 tuổi, được chẩn đoán dị tật Abernethy (tĩnh mạch cửa đổ trực tiếp vào tĩnh mạch chủ dưới) và khối u gan (tăng sản dạng nốt khu trú). Ghép gan từ người cho sống được thực hiện với mảnh ghép gan phải lấy từ mẹ. Sau phẫu thuật, bệnh nhân có chức năng gan tốt và đã trở lại cuộc sống bình thường sau 11 tháng theo dõi. Kết luận: Ghép gan là phương pháp điều trị hiệu quả cho dị tật Abernethy kết hợp với khối u gan.

Summary

Congenital absence of the portal vein (CAPV), also known as Abernethy malformation, is a rare malformation caused by dysplasia in the portal vein system. There are some reports of liver transplantation as a treatment for Abernethy malformation, and our report is the first case in Vietnam. Case report: The patient was a 10-year-old boy, diagnosed with Abernethy malformation (portal vein draining directly into the inferior vena cava) and liver tumor (focal nodular hyperplasia). Living donor liver transplantation was performed using a right lobe graft from his mother. Postoperatively, the patient attained optimal liver function and at 11-month follow-up has returned to normal life. Conclusion: Liver transplantation is an effective treatment for Abernethy malformation combined with liver tumor.
